CODICIL TO WILL
I, ________________, of ________________, being of sound mind and legal age, declare this to be the ________________ to my Last Will and Testament dated ________________, made on ________________.
1. CONFIRMATION OF WILL. I confirm my Last Will and Testament dated ________________ (the "Will") in every respect except as changed by this Codicil.
2. CHANGES TO THE WILL. I amend my Will as follows: ________________.
3. NO OTHER CHANGES. In all other respects I ratify and republish my Will as originally written. Where any part of this Codicil conflicts with the Will, the terms of this Codicil control.
4. INCORPORATION. This Codicil shall be read together with the Will as a single instrument, and the two shall be interpreted to give effect to my wishes as expressed in both documents.
5. GOVERNING LAW. This Codicil is governed by the laws of the State of ________________. If any provision is found unenforceable, the remaining provisions of this Codicil and of the Will stay in full effect.
6. EXECUTION. I sign this Codicil as my free and voluntary act, in the presence of the witnesses below, who sign in my presence and in the presence of each other. The witnesses declare that the Testator appears to be of sound mind and under no undue influence.
