For good and valuable consideration, I, ________________, grant to ________________ and ________________ (together, the "Releasees") the right and permission described below, effective ________________.
1. GRANT OF RIGHTS. I grant the Releasees the right to use my name, likeness, image, voice, and any photographs or video recordings of me for the following purposes: ________________. The material covered by this release is described as: ________________.
2. SCOPE OF USE. This permission includes the right to copy, edit, crop, publish, display, and distribute the images in any medium now known or later developed, worldwide, without limitation of time, unless otherwise agreed in writing. I waive any right to inspect or approve the finished use of the images.
3. COMPENSATION. ________________
4. RELEASE OF CLAIMS. I release the Releasees and anyone acting with their permission from any claims arising out of the use of the images, including claims for invasion of privacy, defamation, or violation of a right of publicity.
5. MINORS. ________________ If the person photographed is a minor, the parent or guardian signing below represents that they have the authority to consent to this release on the minor's behalf.
6. AGREEMENT. I have read this release, understand it, and sign it freely. This is the entire agreement about the use of the images and may be changed only in writing.
