RELEASE OF LIABILITY

This Release of Liability (this "Release") is made on ________________ by ________________ ("Releasor") in favor of ________________ (the "Releasee"), in connection with the following activity, event, or matter: ________________.

1. CONSIDERATION AND ASSUMPTION OF RISK. In exchange for ________________, which the Releasor accepts as good and valuable consideration, the Releasor understands and voluntarily accepts all risks connected with the matter described above, including the risk of physical injury, illness, property damage, and in rare cases death, whether those risks are known or unknown.

2. RELEASE OF LIABILITY. To the fullest extent permitted by law, the Releasor releases, waives, and discharges the Releasee and its owners, officers, employees, agents, and volunteers from any and all claims, demands, damages, and causes of action of every kind, whether known or unknown, arising out of or related to the matter described above, including claims based on the ordinary negligence of the Releasee.

3. HOLD HARMLESS AND INDEMNIFICATION. The Releasor agrees to indemnify, defend, and hold harmless the Releasee from any loss, liability, damage, or cost, including reasonable attorney fees, that the Releasee incurs as a result of the Releasor's participation in the matter or the Releasor's breach of this Release.

4. MINORS. If the Releasor is under the age of 18, the parent or guardian named here, ________________, signs on the Releasor's behalf, accepts the risks described above, and agrees to release and hold harmless the Releasee to the same extent as the Releasor.

5. GOVERNING LAW AND ACKNOWLEDGMENT. This Release is governed by the laws of the State of ________________. If any provision is found unenforceable, the remaining provisions stay in full effect. The Releasor has read this Release, understands that it gives up substantial legal rights, and signs it freely and voluntarily.
